Job Description

Job Overview:We are seeking a dedicated and experienced Licensed Clinical Case Manager to join our team. In this role, you will be responsible for coordinating and managing population health programs, ensuring members receive the highest quality care. This fully remote position offers flexible scheduling and the opportunity to make a meaningful impact on members' health and well-being.

What You'll Be Doing:

Pull, sort, and analyze data to determine member eligibility for the Population Health Management Program.

Provide timely, effective, equitable, safe, and member-centric care while following HMO processes.

Manage case assignments, including outreach, documentation, monitoring case progression, and case closure.

Meet reporting and documentation standards while collaborating with department staff and clients.

Assist members in achieving health and wellness goals by addressing barriers, social determinants, motivators, and psychosocial challenges.

Educate members on navigating the healthcare system, ensuring they make informed decisions, and promoting quality, cost-effective outcomes.

Support the operational aspects of the division to meet customer satisfaction and organizational goals.

Maintain confidentiality regarding all computer programs, medical records, and data.

Participate in Quality Management (QM) and Utilization Management (UM) committee meetings, which may require in-person attendance.

Rotate in off-hour/weekend calls, if applicable.

Engage in ongoing professional development to stay current with case management standards and licensure requirements.

Fulfill additional responsibilities as assigned based on program updates and requirements.

What You'll Need:

Active and unrestricted IL LCSW, LCPC, or LPC license.

At least five years of experience in diverse healthcare settings or a Managed Care Organization.

Solid understanding of case management principles, healthcare management, and reimbursement models, with experience in motivational interviewing.

Strong clinical judgment and excellent verbal and written communication skills.

Outstanding organizational, problem-solving, and time management abilities.

Ability to work independently in a fast-paced remote environment while multitasking effectively.

Proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook, PDF) and experience with multiple EMRs.

A high-speed, secure home internet connection and a private home office with a locking door for security and privacy. Backup internet connection options may be required.

Compensation & Schedule:

Salary: $70,000 per year, paid bi-weekly.

Schedule: Flexible daytime hours, including the option of four 10-hour shifts (Monday-Friday, 6 AM-6 PM CT). Flexibility is available after a successful orientation and supervisor evaluation.
